Vision
Locating an In-Network Provider
To locate a participating EyeMed provider, visit www.eyemedvisioncare.com and choose "Find a Vision Provider," and a network drop down box will display. Select the "Insight Network," enter your location, and click submit.

Critical Illness
How to File a Wellness Claim
You can file a claim by calling Voya at the number, or online at www.voya.com/claims. The necessary information includes SSN, Provider Name, date of testing, and type of testing done.
- Go to www.voya.com/claims and scroll down to the “Have a Wellness Benefit Claim?” section and click the “Submit your claim” button.
- Select Critical Illness Insurance.
- Click “Continue” and follow the screen prompts. Once all questions are answered, click “Submit”.
- Your Group Name is: Dalton Public Schools
- Your Group Number is: 0071264-7

Life Insurance
Portability and Conversion Forms
You have the opportunity to continue your life insurance benefit should you terminate employment with Dalton Public Schools. You may either port your policy to an individual term life plan or convert your policy to a whole life plan upon termination. Additional information regarding these two options may be obtained by contacting Reliance Standard.
To apply, submit the Portability or Conversion forms to Reliance Standard within 31 days of your employment termination date in order to ensure your coverage can continue.
